Employee Benefits Information
New Portland
State University employees should use this information as a guide
to
enrolling for benefits. If you have questions about any forms
or benefits plans, talk with a PSU benefits representative by calling

725-4926 or a PEBB Benefits Counselor by calling (503) 378-1102 (Salem)
or (800) 788- 0520(outside Salem).
Medical
and Dental Insurance
- You
must enroll for these benefits within 60 days following
your date of hire.
- Coverage
is effective the first day of the month following
your hire date and your agencys receipt of
the completed enrollment forms.
Life
Insurance and Accidental Death & Dismemberment Insurance
- You will
automatically be enrolled for the required Basic Life Insurance benefit
of $5,000.
- You
may enroll in Optional Employee Life and Optional Spouse
or Domestic Partner life Insurance plans within 60 days
following your date of hire.
- You
are eligible to enroll for $20,000 of guaranteed-issue
life insurance for you and your spouse or domestic partner
within 60 days following your date of hire. A spouse or
domestic partner cannot apply for life insurance unless
the employee is requesting at least the $20,000 guarantee
issue. If you wish to enroll for amounts in excess of $20,000,
you must complete a Medical History Statement, which must
be reviewed and approved by The Standard Insurance Company.
- Coverage
is effective for guarantee issue the first day of the month
following your hire date and your agencys receipt
of the completed enrollment form. The effective date of
coverage for amounts over the guarantee issue is the first
of the month following approval by The Standard Insurance
Company.
- Accidental
Death & Dismemberment (AD&D) Insurance enrollment
does not require a Medical History Statement.
Short
and Long Term Disability Insurance
- You
may enroll for Short and/or Long Term Disability Insurance
within 60 days following your date of hire.
- If
you do not enroll as a new employee, you may enroll for
Short or Long Term Disability Insurance during Open Enrollment
or following and consistent with a qualified family status
change event.
Dependent
Care Flexible Spending Account
- You
may enroll for the pretax Dependent Care Flexible Spending
Account (FSA) within 60 days following your date of hire.
- NOTE:
9-month employees (includes classified academic-year employees)
not enrolled in the twelve month payroll redistribution
plan will have a reduced amount of deductions taken for
the 2002 plan year.
- You
cannot claim expenses for any time period that you are
not working.
- If
you do not enroll as a new employee, you may enroll for
a Dependent Care FSA during Open Enrollment or following
and consistent with a qualified family status change event.
Long
Term Care Insurance
- You
may enroll for guarantee issue coverage within 60 days
following your date of hire. Guarantee issue coverage is
effective on the first of the month following the date
on the enrollment form. The long term care program is available
for enrollment any time during the year, however after
your first 60 days all coverage is subject to approval.
- You
may enroll in coverage above guarantee issue by completing
the Evidence of Insurability form, which must be approved
by Unum Insurance Company. If approved, coverage is effective
the first of the month following the date of approval by
Unum.
- Eligible
family members may enroll for Unum Long Term Care Insurance
by submitting the enrollment application and a completed
Evidence of Insurability form to Unum Insurance Company.
There is no guarantee issue for family members.
